*The Wastewater Operator Certification test fee MUST be PAID DIRECTLY TO THE STATE. The fee listed on the registration form in this booklet is FOR THE CLASS ONLY. You must submit your exam application, citizenship ID form, and $50 fee to the DWQ BY AUGUST 4th to take the exam. Call Judy Etherington to register for the exam (801-536-4344). On-site test registrations are NOT available.
